Online GP Appendix

These terms are applicable to the Online GP benefit only. This appendix forms part of and is subject to the terms and conditions of the Perkbox Standard Terms and Conditions of Supply. Words and phrases defined in the Perkbox Standard Terms and Conditions of Supply shall be given the same meaning in this appendix except where stated otherwise.

The following definitions shall apply to this appendix:

  1. End Users can book consultations (being either a video or telephone consultation) via the Perkbox Platform.

  2. Once a consultation is booked, the End User shall be sent an email to confirm the appointment and provide the access hyperlink to the video or telephone consultation.

  3. The appointment date will be made available to the End Users via the Perkbox Platform.

  4. All consultations are securely recorded in the interests of all parties.

  5. End Users may only access the Online GP service if they are a citizen or resident of the UK. End Users must reside in the UK to access the service.

  6. Hours of operation for video or telephone consultations are shown as available consultation slots and will be no less than 8am – 10pm Monday to Friday.

  7. Due to limitations of telemedicine in safely diagnosing or treating conditions influenced by pregnancy, unless otherwise provided by the Online GP clinician, medical advice or diagnosis will not be provided for conditions that may be affected by pregnancy.

  8. Where appropriate and with the End Users consent and in accordance with relevant Data Protection Laws, the Online GP provider shall ensure that clinicians notify and provide a brief report of any consultation with an End User to their National Health Service GP.

  9. Where appropriate, the Clinician can also provide an open specialist referral or medical certificate to the End User, that will be sent as a password protected electronic document to the End User’s registered e-mail address.

  10. The Online GP provider reserves the right to terminate the provision of services to an End User without notice if any abuse or inappropriate use of any of the services by an End User, may that be verbal or clinical.

  11. The Online GP will include access to telephone and video consultation and private prescriptions where relevant.

  12. Unless otherwise notified, Perkbox will increase the Online GP Charges by the Consumer Price Index (CPI) or 3%, whichever is greater, at the end of the Initial Term or Renewal Term.

  13. Perkbox may, at its sole discretion, provide additional related services under the Online GP Benefit. Such services may be removed at Perkbox’s absolute discretion. If such services were provided at an additional cost, a pro rata refund will be provided to the Customer at the Customer’s request.

  14. In the event of termination of an agreement between Perkbox and an Online GP provider, Perkbox will use reasonable endeavours to procure a replacement provider, but Perkbox cannot guarantee uninterrupted access to the Online GP Benefit in such circumstances. Perkbox reserves the right to terminate it’s the Online GP Benefit on 30 days’ notice in writing given to the Customer in the circumstances where Perkbox is unable (acting reasonably) to procure the services of a replacement provider to provide the Online GP Benefit.
